​CHART's mission is to provide the highest quality of physical rehabilitation programs/services which are individually designed to ensure optimal recovery for patients from injuries/disabilities.

Early Intervention Program

Acute Care
(0-4 weeks post-injury)
3-5 days/week
Modalities, education, Activities of Daily Living (ADL), postural training, gentle reactivation, as appropriate

This program is designed to prevent development of chronic disablility by providing early and consistent pain relief, educating the patient about their condition, and encouraging a gradual return to pre-injury activities, as appropriate. Symptom management and return to full function are primary considerations.

Active Rehabilitation Program

Sub-Acute/Chronic Injuries
(4-6 weeks post-injury)
3-5 days/week
May include therapeutic reconditioning exercises and ADL/functional activities. Prescribed modalities applied as indicated.

This is an intensive, goal-oriented program specifically designed to restore the injured individual's strength, endurance, flexibility, motor control, and cardiopulmonary capacity related to the performance of ADL and general functional skills. Therapeutic exercise and functional training procedures are utilized as appropriate.

Work Conditioning Program

Sub-Acute work injuries
(4-6 weeks)
and Chronic work injuries
(>6 weeks)
3-5 days/week, 2-4 hours/visit
Includes work simulation, therapeutic reconditioning exercises, ADL, patient education in injury prevention, and safe job performance.

This is an intensive, goal-oriented program specifically designed to restore the injured individual's strength, endurance, flexibility, motor control, and cardiopulmonary capacity related to the performance of work tasks, ADL, and general functional skills.

Hand Therapy Program

1-3 days/week
Customized hand therapy programs for a variety of injuries. CHART therapists develop specialized treatment programs of therapy, education, and exercise to restore function and decrease pain.

This program encompasses dynamic or static splinting, modalities, therapeutic hand exercises, functional activities, ADL, biomechanical and sensory assessments, desensitization, and home exercise programs, as applicable.
