CHART has provided patients with high-quality, individually designed physical and occupational therapy programs for more than 40 years. Our therapists will treat your work-related, auto, hand, athletic, personal, and overuse injuries with personalized care to ensure optimal recovery.

Early Intervention

Prevent the development of a chronic disability! This acute-care program provides early and consistent pain relief, and helps you better understand your condition while encouraging a gradual return to pre-injury activities. Modalities, education in Activities of Daily Living (ADL), postural training, and gentle reactivation are incorporated as needed. Therapy is usually two to five days per week and can start from the day of injury to four weeks post-injury. Managing your symptoms and helping you return to full function are the main considerations.


Active Rehabilitation

Ready to bounce back from sub-acute/chronic injuries? This program may include therapeutic exercises, education in Activities of Daily Living (ADL) and functional activities to help you get on the road to recovery. Modalities are applied as prescribed.  


Work Conditioning

Restore your strength, endurance, flexibility, motor control, functional mobility, and cardiopulmonary capacity related to the performance of Activities of Daily Living (ADL) and general functional skills. This intensive, goal-oriented program was created for sub-acute and chronic work injuries. Therapy is usually two to five days per week. Treatment includes work simulation, therapeutic exercises, training in Activities of Daily Living, patient education in injury prevention, and safe job performance.


Massage Therapy

Our medical message is performed by a licensed massage therapist to help improve circulation, relieve pain, and reduce stress. This therapy is available to both Workers’ Compensation and no-fault patients.  


Hand Therapy

Our customized, hand therapy treatment program includes modalities, therapeutic hand exercises, functional activities, ADL, biomechanical and sensory assessments, desensitization, home exercise programs, and static splinting as needed for one to three days per week. Individualized plans treat a variety of injuries to restore your function and decrease pain.
